Overview

Use this screen to enter the general, representative / appointee, and tax matters information to complete the state power of attorney. If the federal
POA
 screen is completed, UltraTax CS transfers the federal representative data from the federal
POA
 screen to this screen. Transferred data may be changed, as needed. Enter data in the Tax Matters Information and Authorization section on this screen to generate the state power of attorney form.

General Information

Enter taxpayer details for the Form N-848, Power of Attorney.

Representative/Appointee Information

Enter representative information in this section. Representative information will transfer from the federal
POA
 screen.
Hawaii-issued state tax identification number or SSN needs to be entered in the statement for each representative. Enter the VPID or TMRID with letters and dashes.

Only the first four representatives in the statement will be used on Form N-848.

Tax Matters Information

Entries made in these fields are used to generate the state power of attorney form.
Hawaii Tax I.D. Number
Enter Hawaii-issued state tax identification numbers in these fields. Be sure to include letters and dashes in the data entry. An example of the format is as follows:
GE-123-456-7890-01
The first two characters represent the account type:
  • GE for general excise
  • TA for transient accommodations
  • RV for rental vehicle
  • SC for sellers collection
  • UO for use only

Acts Authorized or Deleted

Mark the boxes to authorize additional acts and acts specifically not authorized.
