E-file signature documents

Per Pennsylvania Department of Revenue (DOR) instructions, when you file a Pennsylvania return electronically, Form PA-8453 prints when you don't use a PIN as a signature. Form PA-8453 will also print if you're filing a Pennsylvania return as a state-only return (without a federal return attached), and using the federal self-select PIN program to sign the federal return. Form PA-8453 also includes electronic funds withdrawal and direct deposit information.
A jurat/disclosure statement prints when you use the self-select PIN program and file the Pennsylvania return with the federal return.
Form PA-8879 will print when you use the Practitioner PIN program or the ERO enters the taxpayer's PIN.
note
  • Self-selected PINs aren't considered valid signatures on amended returns.
  • You don't need to send these forms to the Pennsylvania DOR. The EROs and transmitters need to retain them for 3 years after the due date of the return, or the date of e-file, whichever happens later.
