Enter the fiduciary name and title for Form 66. If this field is blank, the fiduciary name from the federal 1041 screen displays on Form 66.
Name control if different from federal Screen 1041
If the name control included in scan lines on Form(s) 41ES for extension or estimate, or ID-VP for the return payment, needs to be different from the federal name control, or if the return is not marked for electronic filing, enter the name control.
Use special allocations for beneficiary amounts
If the application allocates income to beneficiaries by specific income types on the federal return, enter
