Initial year of election cycle
Enter the beginning and ending dates for the initial year electing MSP calculation.
Year of election
Select the year the corporation is electing the multistate service provider cycle. This will complete only the Schedule MSP, Part B calculation.
Arizona campus with more than 2000 students
Mark this checkbox if the corporation is a regionally accredited institution of higher education with at least 1 university campus in Arizona that has more than 2,000 students residing on campus.
Qualified to make MSP Election
Mark this checkbox if the corporation is qualified to make the multistate service provider election. This will complete Schedule MSP even if Schedule MSP, Part A - Line A4 and A5 are no.
