The application uses the amount from Schedule K-57, line 18 as an Other addition on Forms K-120 and K-120S. Use this box to enter a different amount.
Computation of Eligible Employee Amount
Number of Employees / First 12 Months / … / Next 12 Months
If the plan began after December 31, 2004, complete all four columns of information to complete Schedule I. If the plan began prior to January 1, 2005, enter only the number of eligible employees for the month (the first column) and the application will automatically complete the maximum amounts for Schedule II.
