Use this dialog to enter a preparer's representative information for use on Form 2848 (Power of Attorney and Declaration of Representative) and Form 8821 (Tax Information Authorization).
To open this dialog, select the Representative button on the Preparer Information dialog.
Fields & buttons
Address, city, state, and zip code
Use these fields to enter the address of the representative. This information defaults to the firm's address. Mark the
New
checkbox if the information entered in the Address
field has changed since the last time Form 2848 or Form 8821 was filed by this representative.CAF number
Enter the representative's IRS-issued CAF number in this field.
Telephone number
Use this field to enter the phone number of the representative. By default, this field contains the firm's phone number as entered in the Setup, System Configuration, then Firm tab.
Fax number
Use this field to enter the fax number for the representative. By default, this field contains the firm's fax number as entered in the Setup, System Configuration, then Firm tab.
Designation
Use this drop-down list to select the appropriate designation for the representative.
Jurisdiction
Use this field to enter the jurisdiction of the representative.
License/Bar/Enrollment number
Use this field to enter the appropriate license, bar, or enrollment number for the current representative.
