You can view the 1095-C forms that you agreed to get electronically from your employer.
- Select theMy 1095sin the Payroll section of the navigation pane.
- If this is the 1st time you've accessed this screen, selectI Agreeif you consent to get an electronic 1095-C form from your employer.
- Any received 1095-C forms are shown on this screen. Select a document to open a PDF version.noteYou'll need to have Adobe Reader version 9.0 or later installed to view or print Forms 1095-C. If you don’t have the current version or if you have problems opening the forms, you can download and install the latest version of the Reader from the Adobe Reader page of the Adobe website.
