Ensure your fillable PDF is correct

Under
Forms & Signatures
, select
Prepare Form
. You may need to scroll down to see this.

You can use the form tools located on top to add form fields.

Each field displays a name.

You can also see a list of fields and their names under
FIELDS
.

You can press
F2
on your keyboard to edit the name.
note
Due to an open issue with displaying filled data, avoid using field names which contain the word “Yes”.

You can also right-click the field, then select
Rename
.

Once you have added the fillable fields. Go to
File
, then
Save
.
